Proof of Residential Address - List of Acceptable Documents
Two acceptable proofs of residential address for Class E & ID card holders must be in applicants name.
- Household information document- Deed, mortgage, monthly mortgage statement, or residential rental/lease agreement
- Florida Voter Registration Card
- Valid Florida Vehicle/Vessel Registration or Title
- Utilities bills – (Ex. water, gas, electricity, cell phone, cable) (must be dated within the last 30 days)
- Automobile Payment Booklet (must be dated within the last 30 days)
- Selective Service card (must be dated within the last 30 days)
- Medical or Health card and/or medical bill (must be dated within the last 30 days)
- Homeowner’s and/or Automobile insurance policy or bill (must be dated within the last 30 days)
- Employer Documentation (W-2 form or 1099 form, paycheck stub) (must be dated within the last 30 days)
- A letter from a homeless shelter, transitional service provider, or half-way house verifying that the customer resides at the shelter address (must be dated within the last 30 days)
- Financial Statements (bank, credit cards or investment accounts) (must be dated within the last 30 days)
- Mail/Documents issued by Federal, State, County or City government agencies (including city and county agencies) (must be dated within the last 30 days)
- Sexual Offender/Sexual Predator/Career Offender: (must be dated within the last 30 days) Certification of Address, section A, and the FDLE Registration form completed by the local sheriff’s department.
This is a fluid list and regular updates to the items that comprise it may occur.
Parents/Guardians of a minor must provide two proofs of address in their name from the list above.
Class E or ID card holders, excluding minors, with no proof of residential address documents - A person who cannot provide two (2) documents that show their name, and current residential address may provide a completed Certification of Address form and be accompanied by the person they are residing with.
The person the customer is residing with must sign the Certification of Address form certifying that the customer resides with them. They must also provide two proof of address documents. Certifier must provide one (1) household information document with the residential address in the name of the certifier: Deed, Mortgage Agreement, recent monthly statement or rental or lease agreement
AND
Must provide one(1) supporting document with the residential address in the name of the certifier: Florida Voter Registration card, Professional License issued by a U.S. Government Agency, Selective Service Card dated within last 30 days, Employer documentation dated within 30 days (Ex. paycheck), Florida Vehicle or Vessel Documentation stub, W-2 Form, Utility bills dated within the last 30 days, Insurance Policy or Bill for Homeowner's and/or automobile dated (Ex. Water, Gas, Electricity, Telephone/Cell, Cable, ETC.) Within the last 30 days, financial statements dated within the documents issued by Federal, State, County or City last 30 days (Ex. Bank, Credit Card or Investment Account Statements), Government Agencies dated within the last 30 days, or FDLE Registration Form completed by the local Sheriff's Department or Department of Corrections dated within the last 30 days.
